Low Vision Assesments

You are entitled to a free low vision assessment if
* You live in Derbyshire
* Your GP is based in Derbyshire
* You have had a sight test in the last 12 months and your vision can no longer be improved by the use of spectacles
Who can arrange an LVA assessment?
* Your hospital eye consultant or a member of their team
* Your GP or Practice Nurse
* Your Optician or Optometrist
* A representative of the D.A.B at the eye clinic which you attend
* You or your carer, by contacting D.A.B in person, on 01332 292262 or email helenroe@dab.org.uk
What happens before the assessment?
Following your referral D.A.B will pass your details on to the optometrist or LVA assessor who covers the area in your live. The optometrist or assessor will contact you to arrange the appointment for the first assessment. This often will be carried out in your own home.
